3 perio maintenance cleanings
2 fluoride treatments

50% off additional regular cleanings
15% off deep cleanings (scaling & root planing)
Comprehensive x-rays & routine bitewing x-rays in each subsequent year
15% off of additional treatments (except clear aligner therapy)
Family Plan

Billed annually for each additional family member (must be a spouse or child to an existing member).
